What you will do:
* Conduct comprehensive assessments of clients referred to the Community Mental Health Team, with a focus on delivering person‑centred and holistic care.
* Develop and implement care plans tailor‑made to meet each client’s unique needs, collaborating with multi‑disciplinary teams.
* Support and intervene for individuals experiencing mental health crises, ensuring they receive timely and appropriate care.
* Liaise with families, caregivers, and external agencies to ensure coordinated and cohesive support networks for clients.
* Maintain accurate and up‑to‑date records, ensuring all documentation complies with legal requirements and best practice standards.
To be successful in this role, you will need 5 years of post‑qualification experience, with evidence that the Assessed and Supported Year in Employment (ASYE) was completed within two years of qualifying. You must have at least 2 years’ experience as a Social Worker (non‑agency) specifically within a mental health team, with particular value placed on experience in an Older Adults Community Mental Health Team. A driver’s licence is required, and you must be able to start within 4 weeks with 3 years of sound references and no gaps in employment.
