We're looking for someone with those skills to join us, working at Conquest Hospital.
This is an exciting time to join JCR. The JCR In Reach service has been a successful addition to JCR. The small team are based in the hospital and attend various meetings to help identify the right people to go home from hospital with JCR.
JCR is a service that supports individuals to regain their independence following an accident or a period of illness, and we help them to remain in their own home. The person will have goals to achieve at home over a short timeframe and JCR supports rehabilitating them to ideally get back to how they were before the accident or illness so they can live their life as they wish.
Your role is to work in a fast-paced environment and be willing to communicate calmly and professionally with others to help identify suitable people ready to go home with JCR. You will be responsible for line management and accurately updating electronic records. You will build up good relationships with various professionals in the hospital.
The team have received excellent feedback for their work to date and you will feel rewarded when you are part of this successful team achieving fantastic outcomes. Note: The working pattern of the role is predominantly 8:30am-5:00pm Monday to Friday but in the future you will be part of a rota where occasional working at the weekend is required.
