Your Responsibilities
* Conduct Care Act-compliant assessments to determine support needs and strengths.
* Manage a diverse caseload, including safeguarding matters, promoting independence and well-being.
* Work collaboratively with families, carers, health professionals, and community organisations.
* Provide advice, address safeguarding concerns, and connect individuals with appropriate services.
* Apply a solution-focused approach to guide decision-making and care planning.
* Ensure accurate and timely documentation in line with statutory requirements and best practice.
Candidate Requirements
* SWE registration and a recognised Social Work qualification
* Minimum of two years’ post-qualification experience
* Confidence in undertaking mental capacity assessments and safeguarding work
* A proactive, community-focused mindset with excellent interpersonal skills
