Main duties of the job

We are seeking to recruit a dynamic and innovative nurse with experience in Paediatric Asthma to join our Patch CCN team for a fixed term of 12 months to cover maternity leave.

As a Clinical Nurse Specialist, you will be an integral part of the multi-disciplinary and Patch CCN team that sits within the wider Children’s community nursing service, working together to deliver this specialist service to children and families across the primary/secondary care interface. You will bring excellent communication, clinical skills and a desire to maintain and role model a culture of continuous improvement and innovation to inspire our workforce.

You will be responsible for developing and delivering specialist pathways of care as part of a multi professional team, working with care providers within Lambeth and Southwark and wider South-East London ICS to ensure children and families receive high quality Paediatric Asthma care in the most appropriate setting.

Detailed job description and main responsibilities

In partnership with members of the multidisciplinary team the post holder will develop, implement and evaluate a seamless specialist service for children with Asthma ensuring that patients receive the highest standard of clinical care.

They will act as a lead specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to health care professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues.

Also ensures the safety and wellbeing of services users, and always working in a manner that promotes dignity and human rights through the adoption of person-centred care principles.

Qualifications/ Education

Essential criteria

1. BSC/1st degree (health related) or equivalent qualification/training and experience
2. Post registration qualification in appropriate area of speciality or equivalent post qualification experience.
3. Leadership course
4. Mentorship course

Desirable criteria

5. To have or working towards a masters degree
6. Diploma in Asthma Care

Experience

Essential criteria

7. Proven evidence of management / leadership skills and autonomous practice
8. Extensive experience of working autonomously at an advanced level within the specialist area delivering effective patient focused care
9. Previous experience of student supervision and clinical education at undergraduate level
10. Experience of audit, research & evidence based care
11. Experience of delivering patient advocacy
12. Experience of delivering change management both personally and as a facilitator

Desirable criteria

13. Experience or participation in service development projects
14. Previous community experience

Skills/ Knowledge/ Ability

Essential criteria

15. Evidence of teaching skills in particular teaching patients in how to manage their conditions and raise their awareness of their condition.
16. Excellent verbal, written, interpersonal & communication skills
17. Advanced clinical, managerial and leadership skills in specialty
18. Able to use skills of history taking and physical assessment to direct patient care
19. Time management and organisation skills
20. Ability to conduct & evaluate own projects successfully
21. Demonstrate an understanding of: the National agenda for speciality. Knowledge of quality standards & NICE recommendations/NSF
22. Ability to carry out audit and research
23. Presentation skills
24. IT Skills

Desirable criteria

25. Undertaken clinical & nursing research
26. An understanding of how mental health impacts on patients with long term conditions
27. Non medical prescribing course.

Professional/ Statutory Registration

Essential criteria

28. RN1 or RN Child Registered Nurse on the NMC register. To hold the appropriate level for the area of speciality.
