Overview
Westrop Medical Practice are seeking to expand our multidisciplinary team by recruiting experienced Visiting Paramedics or Advanced Clinical Practitioners (ACPs) on a bank / zero-hours basis.
The successful candidate will combine home visiting for routine, acute and urgent presentations with providing an element of clinical triage within the practice, helping us to deliver safe, effective, and responsive patient care.

Main duties of the job
* Undertake routine, urgent and same-day home visits to assess, diagnose, and manage patients with acute or chronic conditions.
* Provide an element of triage within the practice to assess clinical needs, prioritise requests, and allocate patients to appropriate services or clinicians.
* Deliver evidence-based care in line with current best practice and local/national guidelines.
* Prescribe and initiate treatment plans (if appropriately qualified and annotated on the professional register).
* Escalate and refer appropriately to GPs or senior clinicians where required.
* Maintain accurate, contemporaneous clinical records in accordance with NMC/HCPC/GMC and NHS standards.
* Contribute to reducing avoidable hospital admissions by supporting patients in the community.
* Work effectively with the wider multidisciplinary team, including GPs, nurses, pharmacists, social prescribers, and allied health professionals.

Purpose of the Role
To provide flexible clinical support through home visiting and in-practice triage. The role ensures patients receive timely assessment, safe management, and appropriate onward referral, supporting the wider multidisciplinary team in delivering high-quality, patient-centered care.
Key Responsibilities
* Home Visiting undertake urgent and same-day visits to assess, diagnose, and manage acute or chronic conditions.
* Clinical Triage provide face-to-face and telephone triage in practice, prioritising and signposting patients to the most appropriate clinician or service.
* Autonomous Practice manage patients independently within scope of practice, escalating to GPs or senior clinicians when necessary.
* Prescribing & Treatment prescribe and initiate care plans (if appropriately qualified).
* Documentation maintain accurate, contemporaneous records using clinical systems (e.g. EMIS/SystmOne).
* Collaborative Care work closely with GPs, nurses, pharmacists, and other team members to deliver safe and effective care.
* Governance adhere to safeguarding, infection control, confidentiality, and professional standards.
Essential Requirements
* Registered with HCPC (Paramedic) or NMC (ACP).
* Experience in urgent care, community, or primary care settings.
* Competence in triage and clinical prioritisation.
* Ability to work independently and as part of a team.
* Flexible, reliable, and able to travel to patient homes.
Desirable:
* Independent Prescribing qualification (V300).
* Experience in GP or PCN settings.
* Knowledge of SystmOne.

Qualifications
* Current HCPC registration (Paramedic) OR NMC registration with MSc in Advanced Clinical Practice (ACP).

* Additional training in minor illness/urgent care.
Experience
* Demonstrable experience in urgent care, community, or primary care settings.
* Experience of autonomous clinical practice, including assessment, diagnosis, and management.
* Experience of triage (face-to-face and/or telephone).
* Previous experience in GP practice or Primary Care Network.
* Familiarity with SystmOne clinical systems.
Skills and Knowledge
* Ability to assess, diagnose, and manage a wide range of acute presentations.
* Strong clinical decision-making and prioritisation skills.
* Excellent communication and interpersonal abilities.
* Ability to work both independently and as part of a team.
* Good IT skills with the ability to use electronic patient record systems.
* Knowledge of local community services and referral pathways.
