Overview

Primary Care Clinical Pharmacist. The closing date is 05 October 2025. North Solihull Primary Care Network is recruiting two Clinical Pharmacists (full time and part time considered) to join six GP practices serving a population of approximately 40,000.

This pivotal role focuses on improving patient care, supporting GP practices, and optimising medicines use across the PCN. You will contribute to delivering high-quality care across the network and may run pharmacist-led clinics and, where appropriate, manage minor illnesses.


Main duties of the job

* Act as a clinical pharmacist within the GP practices in the PCN, working autonomously and travelling between allocated surgeries to ensure equitable access to services. You will be supported by a GP lead for mentorship and development.
* Work as part of a multidisciplinary team, bringing expertise in medicines and prescribing to support holistic care. Team colleagues may include GPs, nurses, HCAs, physiotherapists, paramedics, social prescribing link workers, Health and Wellbeing Coaches, and administrative staff.
* Take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients, including those with complex polypharmacy.

Job responsibilities

* Provide pharmacy support across GP practices within the PCN, with mentorship from a GP Lead or Mentors and GP leads who will discuss role development.
* Collaborate with a multidisciplinary team to improve patient care, contributing medicines expertise toward holistic care.
* Lead and support chronic disease management, undertake clinical medication reviews, and focus on areas such as CVD, COPD, asthma, diabetes, frailty, and related conditions.
* Offer primary support to practice staff for prescription and medication queries, which may include face-to-face, home visits, care homes, telephone, or online consultations. Support repeat prescribing, manage acute requests, and perform medicines reconciliation on transfer of care, while promoting safer prescribing.
* Possibly run minor ailment clinics in surgery as appropriate for a pharmacist-led service.
* Provide clinical leadership on medicines optimisation, contribute to quality improvement, and help manage aspects of the Quality and Outcomes Framework and enhanced services.
* Facilitate integration with community and hospital pharmacies to optimise skill mix, improve outcomes, and enhance access to care.
* Demonstrate motivation and passion to deliver excellent service within general practice; may be supported to develop into a non-medical prescriber role where appropriate.


Person Specification


Professional Registration

* Mandatory registration with General Pharmaceutical Council
* Membership of the Royal Pharmaceutical Society
* Member of or working towards Faculty membership of the Royal Pharmaceutical Society


Qualifications

* Postgraduate diploma level specialist knowledge or equivalent
* Independent prescriber or working towards/intent of gaining independent prescribing qualification


Other

* Safeguarding adult and children level three
* Full UK Driving Licence


Skills, Knowledge and Experience

* Minimum of 2 years post-qualification experience
* Excellent written and verbal communication skills; able to convey complex information understandably to diverse audiences (e.g., patients)
* In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare
* Appreciation of the nature of general practices and primary care prescribing, and strategies to improve prescribing

Salary: Depending on experience, £38,682 to £53,648 per annum pro rata
