Bexley Complex Case Team is seeking a Primary Care Assistant Practitioner to join our multidisciplinary team. The successful candidate will provide clinical support to nurses, occupational therapists, and physiotherapists, delivering high-quality, evidence-based care within a collaborative environment.
The service operates Monday to Friday, 9am – 5pm. The Assistant Practitioner will undertake practical care duties, support planned patient interventions under supervision, and ensure standards of delegated duties are maintained.
The team offers safe, effective care to patients, families, and carers, working in partnership with GPs, social services, voluntary sectors, and other NHS teams. Our services extend across community settings, including patients’ homes, residential, and care homes.

Key Responsibilities
1. Understand and respond to the physical, emotional, physiological, social, cultural, and spiritual needs of patients and carers, implementing appropriate actions under supervision.
2. Manage priorities delegated by the registered nurse/therapist.
3. Perform other duties aligned with the post’s banding, as agreed with line management.
4. Undertake delegated assessment responsibilities and deliver individual care plans to meet patient needs.
5. Participate in delivering complex care as prescribed by healthcare professionals.
6. Document patient progress accurately in the Trust’s information systems.
7. Maintain professional standards of record keeping in line with professional codes and service requirements.
8. Evaluate patient progress and discuss outcomes with senior clinicians in multidisciplinary meetings.
9. Use equipment correctly to enhance patient function and independence.
10. Demonstrate self-care techniques to patients and carers to promote independence.
11. Contribute to service development by identifying improvement opportunities, supported by healthcare professionals.
12. Identify, manage, and act to minimize risks to patients.
13. Ensure the safe and effective use of stock equipment.
