Responsibilities
* Act as a patient's key worker and support coordination of patient care along the treatment pathway.
* Ensure patients are offered holistic needs assessments at relevant points throughout the pathway.
* Assess patient information needs and meet those needs as required.
* Clinically review and assess cancer patients in the Outpatients setting.
* Use professional and clinical skills to act as resource and advocate for patients, promoting holistic wellbeing of patients, family and carers.
* Work as a core member of the multidisciplinary team, supporting the cancer pathway to improve patient experience.
* Deliver evidence‑based individualized patient care according to patients' changing health needs through Holistic Needs Assessment.
Qualifications
* Understanding of the needs of cancer patients.
* Excellent communication skills for advising and supporting patients, relatives and other health professionals.
* Knowledge of managing a range of conditions, including disease‑progression complications and side‑effect management of systemic anti‑cancer treatment and radiotherapy.
* Ability to speak Welsh desirable; Welsh and/or English speakers welcomed.
