Offer description

* Taff Vale practice are looking to recruit an early years pharmacist, up to 2 years registered, working under the supervision and guidance of an experienced pharmacist whilst undertaking the HEIW Post Registration Foundation Programme.
* The role will start in September 2026 at the start of the new programme and we welcome applicants who will qualify in August 2026.
* This includes protected development time and support from a practice supervisor pharmacist who will act as education supervisor, supported by Cardiff University.
* To help to improve the quality and effectiveness of medicines management within the GP practice.
* To provide patient focussed, prudent healthcare.
* To promote medicines safety and review polypharmacy
* To work as a core member of the pharmacy team, working effectively with other health and social care professionals delivering services to patients.
* To support with the education and training of other healthcare professionals, and in particular other members of the practice and pharmacy teams.


Main duties of the job

Clinical responsibilities

* Undertake patient consultations
* General medication and polypharmacy reviews
* Long term conditions compliance, effectiveness, side effects, symptoms, recommending monitoring and changes to medication
* Nursing home patients/housebound patients
* Providing advice to the clinical team regarding medication management
* Liaising with community and health board pharmacists
* Ensuring patients on shared care medications and high-risk medications have appropriate documentation in place and that these are adhered to with recommended monitoring
* Providing counselling and education relating to medication, including polypharmacy review and deprescribing
* Identifying patients with poor compliance or high risk use for clinical review
* Collecting data for audit purposes and in line with LHB incentive schemes
* Compiling and issuing acute and repeat prescriptions

Support the Practice Pharmacist in:

* Reviewing guidance for pharmacists in general practice and recommending changes in practice
* Develop policies and protocols for safe and effective use of medicines, delivering prudent healthcare
* Direct contribution to enhanced service provision

Education & Training:

* Contribute to education and training of the wider MDT
* Support supervision and contribute to providing meaningful experience to undergraduate pharmacist students


About us

Taff Vale is a long established teaching practice covering the Pontypridd and surrounding areas. Our list size is 16,00 patients, with our main branch situated just 15 minutes north of Cardiff

We have 8 GP Partners, 4 salaried GPs, 2 practice nurses, a Clinical Pharmacist, 3 Health Care Assistant. We have associated health professionals who work with us, such as Mental Health Practitioners and Physiotherapists. We have strong and supportive management, administration and reception teams.


Job responsibilities

Patient Focused and Clinical Roles

* Responsible for undertaking systematic medication and polypharmacy reviews, which will take into account social and practical issues for the patient as well as clinical elements.
* To provide medicines optimisation in chronic disease management e.g. heart failure, chronic pain management, AF, respiratory etc.
* To develop pharmaceutical care plans for individual patients as appropriate e.g. targeting those with frequent admissions/exacerbations/frail elderly/ etc.
* Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.
* Ensure patients have appropriate monitoring tests in place when required.
* To undertake medicines reconciliation following admission or discharge and patient moving into/out of care homes and those newly registered with the practice, following up medication related issues with the patient or other professionals.
* To support public health and health promotion activities, including smoking cessation, administration of flu vaccinations, antibiotic awareness campaigns and screening events.
* To resolve and action medication queries from practice staff and patients relating to prescription requests or problems identified through the repeat prescribing service.
* To have direct patient contact which may include dealing with occasionally distressing or emotional circumstances, including contact and terminally ill patients and their relatives or carers.
* To be accountable for own professional actions, in line with local, & national guidelines and medicines legislation.
* Direct contribution to GMS contract performance, and other enhanced service provision (e.g. Shared care LES, Care home DES, anticoagulation LES).

Improving Medicines Management and Safety

* To review clinical correspondence from secondary care to ensure recommendations to initiate new medicines adhere to the formulary and local guidance and take necessary action where appropriate.

To undertake clinical audit relating to medicines

To action requests for acute prescriptions in line with practice protocols

To undertake medicines re-authorisations and review as part of the repeat prescribing system.

To ensure safe, evidence base and cost-effective prescribing in accordance with national and locally agreed standards and priorities, e.g. implementation of NICE guidelines, All Wales Medicines Strategy Group recommendations and Safety Agency alerts and warnings.

To support policies to improve GP practice medicines management services e.g. establish clear drug recall systems to meet monitoring requirements for drugs

To make recommendations on prescribing choices, budge expenditure and risk management where appropriate, without detriment to patient care.

Analyse, interpret and present medicines data to highlight issues and risks to support decision making

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Will be required to contribute to quality improvement and research work/project.

Education and Communication

* To provide professional and expert advice to GPs, nurses, practice staff and patients on all aspects of drug treatment (doses, monitoring, interactions, side-effects etc).
* To develop and provide expert clinical advice to clinical meetings including educational sessions for the practice staff.
* To explore and implement ways to effectively educate and involve patients regarding their medicines.
* Participation in HEIW Post-Registration foundation programme and to build on IP qualification.
* To support the education and training of other healthcare professionals in the practice, including trainee pharmacists, GP registrars and both pharmacy and medical undergraduates.

Partnership Working

* To develop excellent professional links with relevant health and social care colleague, including community pharmacists, domiciliary care teams, care homes, carers, acute care colleagues etc.
* To network with other practice-base pharmacists to share best practice
* To work closely with health board medicines management teams to ensure awareness of local and national priorities and initiatives
* To contribute to the delivery of cluster/collaborative and practice plans.

Person Specification


Experience

* Experience in a range of clinical specialties
* Experience of multidisciplinary working
* Experience of using EMIS


Qualifications

* Masters Degree in Pharmacy or Equivalent
* Registered with GPhC
* Member of Royal Pharmaceutical Society
