We have an exhilarating opportunity for a Social Worker to join the Young Adults Team in Walsall, UK. This ongoing role will have you working full-time from 09:00 AM to 17:30 PM, assisting young individuals aged 18 to 25, primarily those with learning disabilities. What you will do
* Conduct assessments and develop care plans tailored to the individual needs of young adults with learning disabilities.
* Support young adults in accessing necessary services, promoting independence and well-being.
* Collaborate with families, caregivers, and other professionals to implement the best care strategies.
* Ensure compliance with the Mental Capacity Act (MCA) and the Care Act, applying relevant legislation in all interactions.
* Complete and attach the MOC cover form to ensure comprehensive and up-to-date case records.
* Travel within the community when necessary, so a valid driving licence is required.
