The candidateshould demonstrate strong communication/negotiating skills to carry outcomplex clinical telephone triage in a sensitive manner providing assessment,advice and information and be able to work closely with stake holders utilizingprofessional judgement and referring to other agencies/ services whereappropriate.
Main duties of the job
The primary responsibility of the successful candiadate wil be to prioritise incoming calls and direct patients or health professiona;ls to the most appropriate service trying to avoid hospital admission.

Clinical
To provide clinical navigation, and advice to appropriate clinicians
To reassure the rapid and effective transmission of patient data betweenall relevant parties
To deal effectively with safeguarding issues if they arise in line withpolicy.
Establish and maintain strong collaborative arrangements with otherhealth care professionals
Participate in admission avoidance activities
Prescribe medication patients using EPS
Provide advice to professionals aboutalternatives to hospital care, including primary and intermediate health andsocial care (including voluntary) services
Provide brokerage role discussingcapacity with services and liaising with services to free up capacity dependenton wider pressures in the system as part of escalation plans so as to avoid unwarranted admissions.
Make safe, autonomous triage decisionswithout reliance on digital or algorithm based tools.
Managerial
To assist the clinical lead nurse to train, mentor and coach theclinical team where relevant.
To participate in the appraisal process for themselves, and facilitatefor their team
