Reference: f421da28 Meet Poppy-Mae: I am a friendly, outgoing, chatty, easy to get along with, adventurous 15-year-old girl (nearly 16). I enjoy LEGO, going out for food, baking, going to the cinema, arcades, going to the seaside, amusement parks, going to the zoo or farm. I also really enjoy doing crafts like diamond art, felting, textiles. I am 6 years post transplant and take many medications which I can self administer but will need reminding and supervising. I also need eye drops which i cannot yet do myself. Poppy-Mae likes to be called Poppy not Poppy-Mae. My daughter is adventurous and outgoing and out like opportunities to visit various places in the community and develop her confidence and life skills. She would need the person who supports her to be caring and supportive. She enjoys the cinema, animals, chocolate, teddies, and the person supporting her will provide opportunities to build life skills. She has a gastrostomy and epilepsy which the person supporting her will not need to look after but will need to be aware of and will need to have basic training on what signs to look out for if there is a problem and what to do in this instance. You will: · Support Poppy-Mae when out in the community with accessing all the activities she enjoys. · Discuss other activity ideas with Mum to broaden Poppy-Mae’s experiences. · Support Poppy-Mae with daily living tasks such as money management, time management, cooking and baking skills, basic hygiene Extra Information: Mum would like these hours to be used on the weekends and during the school holidays. She is happy to be flexible with what days and hours the PA would like to do in those time frames. I can communicate verbally my wants and needs. I have a gastrostomy. The PA will not need to do anything with the gastrostomy. I have epilepsy which is controlled by medication. The PA will need to have basic knowledge around what to look out for for this. I do not like to go anywhere too busy or loud. Car driver is preferred. Non smoker is preferred. Also, the hours to be used if possible at the weekend, but the family can be flexible with evenings as well. Meet Emily: I am a friendly, easy to get along with, caring and shy 17-year-old girl. I also enjoy just being in bed, scrolling on my phone and watching Netflix. I am almost 1 year post transplant and need medications, water boluses and eye drops. I can manage these myself but do need reminding and will need help with the eyedrops. My daughter is quiet but still wants opportunities to visit various places in the community and develop her confidence and life skills. She would need the person who supports her to be caring and supportive. She enjoys the cinema, bowling and gaming and the person supporting her will provide opportunities to build life skills. She has a gastrostomy which the person supporting her will not need to look after but will need to be aware of and will need to have basic training on what signs to look out for in terms of infection and what to do in this instance. I don’t like loud or startling noises; people crying or being upset. I can become distressed in these situations, so please take me somewhere quiet and offer me some comfort. I can be quite anxious in new places and do not like to go anywhere to busy or loud. I may be wary of new places, so please try and prepare me for this. I am anxious on public transport and do not like to take the bus but it is important for me to learn this skill and become more comfortable with this. Car driver is preferred.
