The Role: Support Worker
Location: Wakefield
Salary: SCP 18 – 22, £30,559 - £32,654 per annum pro rata (Actual £12,223.60 - £13,061.60)
Contract: Initially until September 2026 subject to funding
Hours: 14 hours per week
We will consider 1 person for both of our support worker roles across Leeds and Wakefield and reduce the hours slightly, meaning this could be delivered as one full time role of 35 hours per week.
Key Responsibilities:
* Provide comprehensive case management for people living with HIV
* Develop personalised self-management plans that inspire hope and demonstrate possibilities
* Advocate fearlessly for service users, challenging discrimination and stigma
* Facilitate empowering group sessions and workshops
* Build partnerships across healthcare, housing, benefits, and community services
We're looking for someone who:
* Has worked with diverse, marginalised or disadvantaged communities
* Can develop care and support plans collaboratively with service users
* Has provided advocacy on behalf of vulnerable individuals
* Can design and deliver group activities that engage and inspire
* Is an empathetic communicator who builds trust and rapport easily
* Is digital confident with IT systems and client management software
