About the Role:
Provide high-quality social work support to adults within a hospital environment, facilitating timely and appropriate discharge planning.
Conduct thorough assessments, identifying individual needs and developing person-centered care plans to support safe and effective transitions from hospital to community settings.
Collaborate with multidisciplinary teams, including medical staff, nurses, and other healthcare professionals, to coordinate care and advocate for patient well-being.
Liaise with families and carers, providing support and guidance throughout the hospital stay and discharge process.
Contribute to a supportive and collaborative team environment focused on delivering exceptional patient care.
Requirements:
Registered with Social Care England.
Minimum 3 years' post-qualification experience as a Social Worker.
Experience working within a hospital or acute care setting is preferred.
Excellent communication, assessment, and case management skills.
Strong understanding of health and social care legislation and pathways.
Commitment to person-centered care and ethical practice.
