Transitions Adult Social Care (TASC) Team is within Integrated Adult Social Care and has strong links to Children’s Services and Education. We work with young people from the age of 14 to 18 across the Devon County Council footprint. The team which has grown during its development and comprises of two Team Managers, Advanced Social Worker, Social Workers, Social Care Assessors, Occupational Therapists and Business/Information Support Officers.
The TASC team works to the Care Act principles where transition refers to the planned process of moving a young person to adult services in order to promote independent living skills. We aim to facilitate a young person's ability to live and enjoy their early adult life as independently as possible.
We are looking to recruit a full time Social Worker to join our team and are are looking for someone with experience of working with individuals to promote independence. Someone who is also skilled in partnership working in order to support positive outcomes for individuals.

Social Worker Career Pathway
We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national , which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
