Job Summary
The purpose of the role is to work within the Pharmacy Hub located within Brooklea Health Centre, to support Prescribing Clerks, Clinical Pharmacists and GPs in managing all patient prescription requests for the five constituent practices within Connexus PCN: Birchwood Medical Practice (Brislington), Nightingale Valley Practice (Brislington), Priory Surgery (Knowle), Stockwood Medical Centre (Stockwood) and Wells Road Surgery (Totterdown).
Responsibilities
* Clinical Responsibilities:
o Conduct medicine administration, support medication reviews and medicines reconciliation. When appropriate, utilise consultation skills with patients to ensure they take their medicines correctly and effectively.
o Review clinic letters where medications have changed.
o Medication reviews and medicines reconciliation for new care home patients. Synchronising medicines for transfers between care settings and linking with local pharmacies.
o Management of stock levels of seasonal vaccination (e.g., Flu and Covid-19).
o Maintaining high-risk medication monitoring and reporting.
o Review home blood pressure readings recorded by patients and discuss or initiate treatment when appropriate.
o Provide specialist opinions to address social care needs, offer lifestyle advice, and consult with specialist pharmacists for complex patients.
o Initiate or adjust medication regimes as guided by a GP based on patient blood results (e.g., Lipids, Thyroid).
o Monitor and amend incorrect PPA prescription claims to avoid financial loss for the practice.
o Liaise with patients and local pharmacies to resolve out-of-stock issues and propose alternatives.
o Review patients on oral contraceptives and ensure safe prescribing by reviewing history.
o Support new Pharmacists with clinical or administrative issues.
* Administrative Responsibilities:
o Work closely with the PCN multidisciplinary team for medicine optimisation.
o Supervise Pharmacy Hub with general prescription requests.
o Support and provide training to prescription clerks.
o Develop relationships with other professionals to support integration of the pharmacy team in primary care.
* Management:
o Assist with the management of the Prescription Hub under guidance of the Senior Pharmacist.
o Record and monitor all staff sickness and absences and inform payroll accordingly.
o Manage the Prescription Hub team's day-to-day activities.
o Complete annual appraisals for staff.
o Ability to work without direct supervision.
o A section of the role will be dedicated to supporting the PCN Prescribing Hub at Brooklea Health Centre, with full training provided; previous experience as a Prescriptions Clerk is not essential.
o Process repeat prescriptions daily ensuring compliance with prescribing hub protocols.
o Perform regular searches on drugs requiring monitoring to minimise adverse effects.
o Ensure all current medications have relevant, current authorisations.
o Promote cost-effective prescribing by switching to generic options and using local formulary medication choices where appropriate.
o Handle prescription queries from GPs, nurses, pharmacists, nursing home staff and reception/administration staff.
o Manage medication changes from hospital letters, remove outdated medications, add new or updated entries, and add monitoring diaries.
o Perform drug alert searches and actions.
o Add new patient medications, obtain GP approval when needed, and coordinate recalls and diary entries. Communicate with patients via phone, SMS, or letter, and with pharmacies regarding changes as required.
o Consult with pharmacies regarding monthly dosette box prescriptions.

Qualifications and Experience
* Essential
o Registered with General Pharmaceutical Council (GPhC) as a Pharmacy Technician.
o Experience as a registered pharmacy technician in primary care, hospital, or community pharmacy.
o Knowledge and understanding of pharmacy laws and ethics and current legislation.
* Desirable
o Experience working in a primary care setting.
o Accredited Checking (or nearing completion).
o Medicine Reconciliation (or nearing completion).
Qualifications
* Essential
o Registered with General Pharmaceutical Council (GPhC) as Pharmacy Technician.
* Desirable
o Completion of CPPE Primary care pharmacy education pathway or Medicines Optimisation in Care Homes pathway.
o Enrolled in or qualified from an approved training path.
o Evidence of continuing personal development.
