Currently, Cornwall has one of the highest rates of hospital stays during the last 90 days of life, a clear signal that change is needed. We are creating a more patient‑centred, community‑based approach that brings compassionate care closer to home in the last year of their life or nearing the end of life.

* A 24/7 Rapid Community Response and Care Service
* A dedicated End of Life Care phone line for public and professionals
* Strengthened voluntary sector partnerships and our Hospital@Home virtual service
We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference with a requirement to cover Cornwall and the Isles of Scilly.
Main duties of the job
Assist in the delivery of clinical care, planned and assessed by the Registered HCPs.
Carry out care and other duties under indirect supervision of a Registered HCP.
Support care on a day‑to‑day basis for all patients in the clinical area in a manner sensitive to the individual clients’ needs and wishes, always acknowledging their rights and beliefs.
Contribute to resource management within the clinical area.
Rotationally work within the Telephone response service for Palliative and End of Life Care, providing a 24/7 working closely with admission avoidance services to ensure patient safety is maintained.
Collect any referral information and, with support from the clinical team, assist in directing the caller/referrer to the correct service for their immediate requirements.

Person specification
Education and Qualifications / Experience
* NVQ 3 in Direct Care or working towards it
* Previous clinical experience in a healthcare setting
* Knowledge and experience of providing clinical skills such as venepuncture, aseptic non‑touch technique, basic observations
Skills and Aptitude
* Able to competently organise and carry out designated workload within the scope of previous training
