An exciting opportunity has arisen for an experienced and motivated Nurse Practitioner to join our friendly and supportive team. We are looking for a dedicated professional who can work autonomously and collaboratively to deliver high-quality, person-centred care.
You will play a key role in managing long-term conditions including:
* Diabetes
* Heart Failure
* COPD
* Asthma
Key Responsibilities
Assess, diagnose, and formulate treatment plans for acute and chronic illnesses
* Conduct telephone and in-person reviews for patients with chronic conditions
* Manage prescriptions, results, and referrals
* Review Medlink questionnaires and interpret test results
* Educate patients on preventative healthcare and lifestyle changes
* Perform core nursing duties including injections and cervical screening
* Collaborate with the wider clinical and administrative team
* Maintain accurate and up-to-date medical records
NMC Registration
Experience in Minor Illness Treatment and Chronic Disease Management
Registered Non-Medical Prescriber
Evidence of continuous professional development
* Excellent communication and interpersonal skills
* Ability to work under pressure and multitask in a busy, changing environment
Desirable Skills & Qualifications
Experience using Medlink
Independent Nurse Prescriber qualification (preferred but not essential)
Strong organisational skills
Ability to work independently and as part of a multidisciplinary team
