* Plan discharges from an acute environment.
Qualifications
* Experience as an Occupational Therapist at Band 6, 7, or 8a level.
* HCPC Registered.
Required Skills
* Experience with complex discharge assessments and planning from an acute environment.
Preferred Skills
* Additional certifications in Occupational Therapy.
* Experience working in multidisciplinary teams.
Full-time, 37.5 hours per week, Monday to Friday. 6-month contract with the possibility of extension.
