Are you passionate about supporting patients to improve their health before surgery? We are seeking a proactive and compassionate individual to join our team as a Health Improvement Advisor (Preoperative Pathways).
This role is part of a wider system shift towards prevention, as outlined in the 10 Year Health Plan: Fit For the Future. You will provide flexible, telephone-based support to patients awaiting surgery, helping them to stop smoking or reduce tobacco-related harm. You’ll also explore opportunities for wider health improvement, using person-centred approaches to advise, signpost, and refer patients to community-based services.
Key Responsibilities:
1. Deliver motivational and behavioural support to patients preoperatively.
2. Develop personalised care plans in partnership with patients.
3. Facilitate access to appropriate health improvement services.
4. Champion the tobacco smoke-free message within the multidisciplinary team.
5. Support the development and daily running of the service.
About You:
6. Currently in a Band 3 patient-facing role and seeking a developmental opportunity.
7. Excellent communication skills with the ability to build trust quickly.
8. Non-judgemental, collaborative, and committed to improving patient outcomes.

9. Deliver telephone-based health improvement advice to preoperative patients using motivational interviewing techniques.
10. Identify and contact patients based on electronic health survey data to offer personalised support.
11. Provide tobacco dependency support, including referrals to stop smoking services and guidance on hospital-based options.
12. Offer general lifestyle advice on alcohol, diet, and physical activity.
13. Maintain accurate patient records and contribute to service monitoring and evaluation.
14. Respond to referrals promptly and escalate concerns to the clinical lead as needed.
15. Collaborate with multidisciplinary teams and contribute to service development and training.
16. Participate in regular supervision, team meetings, and continuous professional development.
