The Cancer Services Team is seeking Cancer Follow-up Coordinators to collaborate with Cancer Support Workers and Clinical Nurse Specialists, ensuring patients receive timely, effective follow-up care after cancer treatment. The roles are based within specific cancer-type teams across all hospital sites, including Royal Sussex County Hospital, Worthing Hospital, and St Richard's Hospital.
Key responsibilities include:
1. Tracking patients post-treatment, including enrollment and discharge from remote follow-up systems.
2. Coordinating investigations and symptom assessment questionnaires as per schedules.
3. Evaluating results within clinically supervised protocols.
4. Communicating review outcomes to patients via standardized letters.
5. Escalating concerns to the clinical team as needed.
