We are looking for dedicated and dynamic Occupational Therapists to join our Community Occupational Therapy Team and our Independent Living Services. These roles provide the opportunity to work closely with individuals in their own homes, supporting them to regain independence, improve their quality of life, and prevent unnecessary hospital admissions. Key Responsibilities: Conduct holistic assessments to identify individual needs and develop person-centred support plans. Recommend home adaptations, equipment, and assistive technology to promote independence. Work collaboratively with multidisciplinary teams, carers, and family members to deliver high-quality interventions. Support hospital discharge planning, preventing readmissions and enabling safe transitions into community settings. Promote strengths-based approaches that empower individuals to lead fulfilling lives. Provide education and training to staff, service users, and carers on occupational therapy interventions. What We’re Looking For: Degree in Occupational Therapy and registration with HCPC. Experience working in a community or social care setting (desirable). Strong assessment skills with the ability to develop creative solutions to enhance independence. Excellent communication and interpersonal skills for working with individuals, families, and professionals. Ability to manage a diverse caseload, balancing urgent needs and long-term interventions.
