Maidenhead Primary Care Network, is a group of 9 GPPractices working together with a range of local providers to offer morepersonalised and coordinated health and social care to the 80000 patientpopulation of Maidenhead in Berkshire.
We are looking to recruit to the post of Health andWell-being Coach, to work within our Primary Care Network multidisciplinaryhealthcare team.
Main duties of the job
Health & wellbeing coaches help their clients to assesstheir lifestyles and wellbeing, set goals for improving their health, agreeaction-plans, and provide practical support. Clients accessing this servicewill be adults and the role will offer a one-to-one short term intervention; aswell as developing groups to support and sustain change. You will work closelywith GPs and practice teams, making sure that appropriate support is madeavailable to people; supporting them to understand and manage their conditionand ensuring their changing needs are addressed. They will enable people toaccess the services and support they require to meet their health and wellbeingneeds, helping to improve peoples quality of life.
You will work alongside social prescribing link workers and care coordiantors to provide an all-encompassing approach topersonalised care and enable people navigate through the health and caresystem.
The post holder will work with a diverse range of peoplefrom different cultural and social backgrounds. The ability to work confidentlyand effectively in a varied, and sometimes challenging environment isessential.
The successful candidate will have excellent interpersonaland communication skills, and be organised, patient and empathetic. They willhave experience of working in health, social care or other support rolesincluding direct contact with people, families or carers
About us
Maidenhead Primary Care Network has been successfullydelivering at scale Care to the population of Maidenhead for 5 years. We have alarge team of both clinical and none clinical staff.
While each Health and Well-being Coach will become embeddedin up to 3 Practices, they frequently enjoy a mutually supportive jointapproach to work with other members of the 20 strong PCN Health and WellbeingTeam know as the PAC Team. The Team benefit from both practice and PCN supervision and training.

Job responsibilities
Health and Wellbeing Coaches support people to takepro-active steps to improve the way they manage their physical and mentalhealth conditions, based on what matters to them. They support people todevelop their knowledge, skills, confidence in managing their health and care,to improve their health outcomes and quality of life and support them makingchanges in health related behaviours. Health coaches do this by coaching andmotivating people through multiple sessions to identify their needs and to setgoals for themselves, and through providing access to interventions such asself-management education and peer support.
Health and Wellbeing Coaches manage and prioritise a caseload,according to the needs, priorities and support required by individuals in thecaseload. They identify when a persons needs are beyond the scope of the healthand wellbeing coach role and refer them back to other health professionals ororganisations. Health and Wellbeing Coaches may work with people by phone, byvideo conference, or face- to-face.
The successful candidate will be kind, reflective andself-aware and will enjoy working with a wide range of people. They will havegood communication and negotiation skills and a firm belief that people haveuntapped resources within them, that can be unleashed by providing highquality, non-judgemental support. They will be proactive with a flexibleattitude, keen to work as part of a team and committed to developing themselvesin this skilled role and supporting colleagues to do the same.
They work closely with GPs and practice teams to manage acaseload of patients, acting as a central point of contact to ensureappropriate support is made available to them and their carers; supporting themto understand and manage their condition and ensuring their changing needs areaddressed.
This is achieved by bringing together all the informationabout a persons identified care and support needs and exploring options to meetthese within a single personalised care and support plan, based on what mattersto the person.
Health and Well-being Coaches review patients needs and helpthem access the services and support they require to understand and managetheir own health and wellbeing, referring to social prescribing link workers,health and wellbeing coaches, and other professionals where appropriate.
The successful candidate will be based in a local cluster ofGeneral Practices as part of the Primary Care Network (PCN). They will becaring, dedicated, reliable and person-focussed and enjoy working with a widerange of people. They will have good written and verbal communication skillsand strong organisational and time management skills. They will be highlymotivated and proactive with a flexible attitude, keen to work and learn aspart of a team and committed to providing people, their families and carerswith high quality support.
This role is intended to become an integral part of the PCNsmultidisciplinary team, working alongside social prescribing link workers andhealth and wellbeing coaches to provide an all-encompassing approach topersonalised care and promoting and embedding the personalised care approachacross the PCN.
There may be a need to work remotely depending on therequirements of the role. Please note that the role of a Health and Well-beingCoach is not a clinical role.

Experience
* Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity) Experience of supporting people, their families and carers in a related role Strong organisational and written skills, including planning, prioritising, time management and record keeping
* Knowledge of Safeguarding Children and Vulnerable Adults policies and processes
