The Crisis Resolution and Home Treatment Team for older adults is a well-established, friendly, and supportive multidisciplinary team based in Fulbourn, Cambridge. The team offers intensive support and treatment at home and in care homes for older adults with mental health challenges, including the behavioral and psychological symptoms of dementia. The services provided include:
* Crisis intervention, intensive treatment, support, risk assessment, and management to prevent unnecessary hospital admissions and facilitate earlier discharge from mental health hospitals.
* Dementia Intensive Support, which offers practical and psycho-educational approaches to care within patients' homes and care homes.
We are a strong, passionate, and fun-loving team with extensive clinical experience. We are looking for a person who shares our passion for working with older people, desires to develop and enhance skills, and can work under pressure in a busy team. The ideal candidate will have experience in clinical assessments, including robust risk assessments, and be a strong team player with a good sense of humor, motivated by making a difference, and capable of contributing to team meetings and leading within the team.
Main duties of the job:
* Provide mental health services within the community seven days a week, including psychosocial assessments for individuals with various mental health issues.
* Work in partnership with service users and carers to deliver treatment in the least restrictive environment.
* Act as a care coordinator, ensuring comprehensive assessment, planning, implementation, and review of individual care needs.
* Assess and provide care/support for those at risk of hospital admission, facilitating admissions and rapid discharges as needed.
* Conduct clinical risk and mental health assessments, formulate care plans, and provide short-term contingency planning during crises.
* Promote effective teamwork and serve as a role model within the service.
* Deliver a crisis and home treatment service, including dementia intensive support.
