Main duties of the job
We are looking for an enthusiastic and committed staff member to join our existing community service. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with the Multi-Disciplinary Team: a spectrum of clinical professionals which includes Nurses, Psychologists, Doctors, Social Workers, Occupational Therapist and support staff. A key focus of the role is that of undertaking physical health assessments.
To work collaboratively with colleagues, service users and their carers to ensure the delivery of focused care and treatment. This role involves working with individuals who are experiencing a range of mental health conditions and who, as a consequence of this, might be vulnerable. A key component of this role is that of supporting with physical health assessments for which training will be provided. You will work as part an established multi-disciplinary team; working under the direction of a qualified practitioner to deliver individualized support in accordance with the agreed care plan.
