Responsibilities
* Act as care/recovery coordinator for a caseload of service users, organising and participating in continuous assessment, planning, delivery and review of care.
* Deliver defined assessment methods and interventions for individual and group activities.
* Arrange CPA meetings, risk assessments, crisis, rapid access and rapid response plans in line with trust policy and local safeguarding procedures.
* Develop and review appropriate care plans, identify and organise other resources, and communicate with service users, carers and multidisciplinary teams.
* Provide therapeutic activities to improve coping skills, sleep, anger, anxiety, stress management, assertiveness, relaxation and physical health such as exercise and nutrition.
* Enhance daily living skills, confidence and independence in line with personal recovery plans.
* Offer emotional support and information on rights under the Mental Health Act.
* Promote individuals’ rights, independence, safety and wellbeing, recognising and reporting discriminatory behaviour.
* Work collaboratively with service users, carers, and staff to manage mental health needs and relapse prevention strategies.
* Respond sensitively to the needs of individuals influenced by age, culture, race, gender, ethnicity, social class or disability.
Qualifications
* NVQ Level 3 in a mental health setting or equivalent qualification.
* Demonstrated understanding of recovery principles, secondary mental health services and relevant legislative frameworks (MHA, CPA).
Experience
* Experience working in any health, social care setting or related voluntary organisation.
* Experience of risk assessment and crisis management in a prison or secure environment.
* Experience of risk and crisis planning, rapid access, safeguarding and discharge planning.
* Experience of theoretical foundations of health and social care frameworks.
* Experience of engagement with carers and supporters.
Skills
* Good communication skills and ability to build positive relationships with patients, carers and professionals.
* IT literacy, confident in using Microsoft Office.
