We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of Triage Clinical Nurse Specialist within our community palliative care team.
This post requires you to act as the first point of clinical contact for the patient, following patient referral. You will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professional.
This is an exciting time to join our team as we look at expanding our care within our community through clinics and expansion of our hospice at home team.
Main duties of the job
1. Work as part of the specialist palliative care community nursing service, triaging referrals, providing telephone advice and support to patients, families, carers, and other health care professionals.
2. To provide telephone advice and support to patients known to the Palliative Care Community Team
3. Attend multidisciplinary team management meetings
4. To work collaboratively with other providers of palliative care to work towards providing equity of care across Buckinghamshire
5. To work as an effective team member within the specialist palliative care service, working collaborating with other specialist palliative care professionals, Primary Care Teams and other sectors including, hospital providers, private sector, voluntary services, social services and any other care service providers.
6. To provide education, advice and support to professionals, patients and all carers working with patients.

Education, Qualifications and Training
Essential criteria
16. Registered nurse with current NMC registration
17. Palliative care degree or working towards
18. Non medical prescribing qualification or commitment to undertake
19. Evidence of continuing professional development
Desirable criteria
20. Advanced communication skills course
Experience
Essential criteria
21. Extensive experience in palliative care setting or related speciality
22. Experience of working in multi-disciplinary teams and other agencies
Desirable criteria
23. Experience of working at A4C Band 6 level
24. Experience of teaching, audit and research
25. Experience of community working
