Occupational Therapist – Community Reablement
Community OT Team – Pathway 1 Reablement (Wolverhampton)
A service in Wolverhampton is seeking an experienced Senior Occupational Therapist to join their Community OT Team supporting Pathway 1 Reablement. This is a 12-week role working 3 days per week, focused on supporting individuals in the community to regain independence following hospital discharge.
Main Duties
* Provide occupational therapy assessments within the Pathway 1 Reablement service
* Support individuals to regain independence at home following hospital discharge
* Carry out functional assessments and develop goal-focused reablement plans
* Recommend appropriate equipment, adaptations, and interventions to promote independent living
* Work collaboratively with multidisciplinary teams including health and social care professionals
* Manage and prioritise a community-based caseload
* Support discharge pathways and help prevent hospital readmissions
* Provide professional advice and guidance to colleagues within the service
Requirements
* Diploma/Degree in Occupational Therapy
* HCPC Registration
* Previous experience working in community or reablement services
* Experience completing functional assessments and supporting independence at home
* Ability to work independently while contributing to a multidisciplinary team
* Strong communication, organisational, and clinical decision-making skills
* Ability to travel within the community as required
