The Hospital at Home Teams within Fife provide a service for the frail elderly population (predominantly over 65yrs), which prevents acute hospital admission or facilitates early supported discharge where acute hospital admission has taken place. It allows people to have a period of “acute” medical care within their own home, which will in turn allow the person to recover from a period of illness without needing hospital admission.
Following a successful test of change between Hospital at Home and our acute services we are excited to appoint 2 Nurse Practitioners to join our well-established Hospital at Home service. These staff will part of the core Hospital at Home Service with the expectation of supporting the in reach work within Victoria Hospital. This will involve being based within the acute hospital as part of a small rotation supporting discharges and carrying out clinical assessments for patients stepping down to the Hospital at Home Service. Out with the time in the acute hospital staff will be expected to work in one of the Hospital at Home teams across Fife.
The successful Nurse Practitioners will be required to deliver a high standard of safe and effective acute medical patient care within mainly the acute setting but also supporting the wider Hospital at Home service in the community as required. They must have the ability to work autonomously in this evolving aspect of the service and must have a qualification at degree level in clinical decision making and non-medical prescribing. The additional skills of venepuncture, ECG, IV additives and cannulation will be required also for this role. Excellent communication, confidence and good degree of assertiveness and time management skills are essential in this role.
The ability to travel within the area in a timely manner is also required to be able to support the wider Hospital at Home service and be able to respond to deteriorating patients and provide daily routine clinical interventions in the community.
