Responsibilities
* Assessments, reviews, care-planning, duty and capacity assessment.
* To practice effectively, exercising high quality judgements in situations of increasing complexity, risk, uncertainty and challenge, acting with confidence and considerable autonomy whilst accessing and using support and advice.
* To support and assist the provision of high-quality adult social care for older people and adults within national and local policy guidelines by assessing and meeting the care needs of adult citizens in the district.
* To develop skills in evaluating the quality and impact (outcomes) of own practice and that of the team and service., Dedicated Recruitment Consultant.
* Aftercare service delivered by Candidate Care Team.
