We are looking for warm, caring individuals to provide person-centred support to service users with complex mental health needs in the Northenden area. Your role will include:
* Maximizing service user outcomes by promoting independence and social inclusion
* Encouraging service users to be involved in decisions regarding their care and lives
* Providing personal care, practical and emotional support to adults with mental health needs
* Collaborate with service users, their families and involved professionals to provide a consistent and coordinated service.
* Develop and maintain health action plans.
* Maximize and develop communication plans for services users.
The Support Living Coordinator role will be to lead the team to provide individualised person-centred support to people with Mental health needs living in their own homes; to enable them to enjoy a fulfilling and valued life, to participate in the community and to develop their abilities as fully as possible.
You will have good leadership skills, a warm, caring nature, and a commitment to making a positive difference in people's lives. It is expected that you will work flexibly, including evenings and weekends, to meet the needs of the service.
