Essex Partnership University Trust (EPUT) is recruiting HCA/Rehabilitation Assistants (Health Care Assistants in our rehab team) for our community rehabilitation service.
The Collaborative Care Team works jointly with Frailty Virtual Ward and Community Stroke Team/Early Support Discharge Service.
Main duties of the job
The team is a multidisciplinary, therapy-led unit providing rapid assessment and rehabilitation for patients in their homes. They support discharge facilitation and aim to prevent hospital admission for conditions including stroke, orthopaedics, unstable fractures, neurological issues, and collaborate with Frailty Virtual Ward services.
As part of a friendly, supportive team, you will provide person-centred care and support recovery. The service covers South East Essex, including Castlepoint, Rochford, and Southend, so applicants must be able to travel efficiently across this area.
The successful candidate will assist with personal care and rehabilitation programmes across various shifts, seven days a week, aligned with service needs. Candidates should be motivated, eager to learn, enthusiastic, and possess good interpersonal skills for communicating with patients, families, carers, and colleagues.

Job responsibilities
The Mid and South Essex ICS unites health, care, community, and voluntary sectors to improve health outcomes and reduce inequalities. Our HCA Academy, a collaborative initiative between EPUT and MSEFT, supports onboarding, induction, education, and experience for Healthcare Assistants.
Successful candidates will attend a 5-day face-to-face Induction Programme at Brentwood Community Hospital, with start dates discussed during recruitment. This programme aims to support early employment stages and enhance skills and knowledge, followed by a 5-day mandatory training at EPUT.
