Croydon Council, Adults Social Care, is seeking to recruit a Health and Wellbeing Officer to work in the Adults Early Intervention Support Service (AEISS), leading the delivery of vital first-point-of-contact services to adults in need of support.
Key responsibilities include:
1. Ensuring timely and efficient responses to referrals and inquiries received.
2. Responding to phone calls and incoming referrals, ensuring individuals receive the right care and advice promptly.
3. Maintaining a customer-focused approach, emphasizing early intervention, prevention, and supporting independence.
4. Providing clear and accessible information and advice to individuals, carers, and families.
5. Working closely with individuals and communities to ensure services are responsive and inclusive, especially for vulnerable or hard-to-reach groups.
