About The Role

When dementia changes someone’s life, the right support can make all the difference. If you care about helping people live well with dementia and making sure carers feel informed, listened to and supported, this could be a role where you genuinely see the impact of your work.

Our Dementia Adviser’s have the rewarding job of providing support and guidance to people with dementia and their carers; helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. Our person-centred service is unique to each person we support as it is based upon their personal circumstances and support needs.

We offer support to clients in a variety of ways, predominantly by delivering Carers Information and Support Programmes, Peer Support and Activity Groups in communities throughout the county, and on occasion covering other activities and provide dementia support, holding a small caseload if required. The successful candidate will join the team with one other Dementia Adviser and you will lead on all group activities being delivered as part of the Age Well contract.

What you’ll focus on:

1. Making sure all groups are high quality and meet the standards and goals set out in our Service Specifications.
2. Manage our Group Support Volunteers who will support you in the delivery of groups.
3. You will recruit and train new volunteers to ensure all groups are properly advertised and staffed
4. You will work from home and in the community, running groups, visiting people at home, promoting our service at events, and occasionally giving talks about dementia.
5. Identifying people’s needs and the services they may wish to access, give advice about how to live well with dementia and support the improvement of their sense of wellbeing
6. Collate statistics and evidenced based outcomes to meet our targets including case studies and evaluation
7. Engaging with a variety of professionals from health, social care, voluntary and community sector.
8. Build relationships to ensure referrals into our service, and you have knowledge of services and support available for people with dementia and carers.

About you

9. Ability and interest in supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives.
10. Good communication skills, adapting to a wide range of audiences and delivering presentations and talks.
11. Understanding of dementia and the needs of those living with dementia and their carers.
12. Able to network effectively and collaborating with other professionals to achieve a positive outcome for people.
13. Good IT skills, including experience working with databases and virtual meeting software (Teams / Zoom).
14. Ability to organise own work to ensure it is accurate, meets quality targets, deadlines and reporting requirements.
