Our Early Intervention Team is one of the top-performing teams in the country. Clinicians in our team hold smaller caseloads that enable them to deliver high-quality care. 1. Take responsibility for managing a complex caseload, including specialist assessments, care planning with service users and carers, and evaluating treatment plans.
2. Participate in setting quality standards, including auditing, monitoring, and reviewing practice according to current clinical guidelines, practice, and policy.
3. Administer medication as appropriate, following Trust procedures and NMC guidance.
4. Assess individual care needs, develop, implement, and evaluate care programs.
5. Deliver interventions in line with care plans and team guidance, including pharmacological and psychologically informed approaches.
6. Complete risk assessments and develop risk management plans, promoting positive risk-taking to enhance quality of life and independence.
7. Maintain effective reporting systems on patient conditions verbally and in writing.
We seek a highly motivated Band 6 Mental Health Practitioner (nurse, occupational therapist, or social worker) to join our early intervention in psychosis team. The role involves delivering evidence-based, compassionate, person-centered care, with an emphasis on assertive outreach. The successful candidate will work closely with a multidisciplinary team to ensure optimal care for individuals experiencing a first episode of psychosis.
The role covers mainly the East of the patch, including Burton, Tamworth, Lichfield, and surrounding areas, with flexibility to travel across South Staffordshire as needed. Flexible working arrangements may be discussed.
