Job Title:

Care Co-ordinator

Responsible to:

Office Manager / Deputy Practice Manager

Hours

37 hours per week - Monday, to Friday

Pay

To be reviewed after successfully passing your 3-month probationary period

Main Duties and Responsibilities

Act as first point of contact for patients to facilitate their access to the range of services provided by the Practice and where appropriate signpost them to the wider healthcare system.

Answer the telephone to patients and other callers.

Greet, assist and direct patients and visitors to the Practice

Co-ordinate the flow of communication between clinicians and patients during the working day. Ensure that messages are accurately delivered to the appropriate person

Pro-actively invite patients to attend appointments, check-up or regular long-term condition clinics as appropriate.

Provide patients with appointment slots that, to the greatest extent possible, meet their needs and optimise the use of clinical time in accordance with practice policies.

Actively promote services to patients and facilitate their access to them.

Ensure patient contacts by phone, digitally, or in person are accurately recorded on the system so that other members of the team have a complete picture of a patients status if they are dealing with them in future.

Provide support to other members of the team as appropriate

Processing requests for medication and resolving queries in line with protocol.

Participate in team meetings and contribute ideas for improving how we provide care

General administration to support the smooth running of the practice.

Qualifications Essential:

Good standard of general education

Experience desirable but not essential as you will be given training in a customer based role.

Skills Essential:

Good level of numeracy and literacy

Excellent communication skills (Written and Oral)

Strong IT skills

Clear & polite manner in face-to-face and phone contact

Ability to plan and organise around priorities and work to complete multiple tasks to strict deadlines

Effective problem solving skills

Good interpersonal skills: especially influencing and empathic listening
