️ Qualified Adults Social Worker – South Integrated Care & Support Team

Are you a passionate and experienced Social Worker looking to make a real impact in adult social care? We’re recruiting on behalf of a well-established local authority in Tameside, seeking a dedicated professional to join their South Integrated Care & Support Team.

The team takes a strengths-based, person-centred approach, promoting independence, safeguarding wellbeing, and supporting people to live safely and with dignity in their communities.
About the Role
As part of the South Integrated Care & Support Team, you’ll work collaboratively with health professionals, community partners, and other agencies to provide holistic and proactive care to adults in the Tameside area.
You’ll manage a varied caseload and play a key role in:
* Carrying out Care Act assessments
* Completing Mental Capacity Assessments
* Leading and coordinating safeguarding enquiries
* Supporting individuals with complex needs to access the right care and support
* Participating in the team’s duty rota and screening incoming referrals
* Promoting independence and wellbeing through creative, outcomes-focused interventions
You will be supported by a strong management structure, regular 1:1 supervision, and opportunities for ongoing professional development within a forward-thinking, supportive local authority.
About You
We’re looking for someone who brings professional expertise, compassion, and a strong understanding of legislation and best practice in adult social care.
You must have:
* A recognised Social Work qualification (SW Degree, DipSW, CQSW or equivalent)
* Registration with Social Work England
* Minimum 2 years' post-qualification experience (4+ years preferred)
* Sound working knowledge of key legislation, including:
o Care Act 2014
o Mental Capacity Act 2005
o Mental Health Act
o Human Rights Act
* Confidence managing complex cases, safeguarding, and capacity-related work
* Strong partnership-working skills and a commitment to multi-agency collaboration
* A reflective, innovative, and solution-focused approach
