1. The post holder will work closely with the Sister/Charge nurse and multi-disciplinary team members in monitoring and maintaining excellent clinical standards of nursing practice and staff development as part of the Transplant Outpatient team.
2. Support the continuing professional development and overall performance of junior staff, in partnership with senior colleagues. Regularly take charge of the clinical area, ensuring safe staffing levels and effective resource utilization. Deliver all aspects of patient care independently, demonstrate clinical procedures, and supervise qualified and unqualified staff while maintaining a safe working environment. Maintain flexibility to provide cover in other clinical areas as appropriate, based on experience. Be accountable for personal nursing practice, comply with Trust and NMC policies and protocols, and take responsibility for ongoing personal and professional development

Main duties of the job
3. The post holder is expected to carry out all relevant forms of care without direct supervision, demonstrate procedures, supervise qualified and/or unqualified staff whilst maintaining a safe working environment
4. Be able to adapt to the staffing needs of the department, providing cover according to level of experience and area specified
5. The post holder will be accountable for their nursing practice, in accordance to the Nursing and Midwifery Council and must adhere to Trust policies and protocols

Working for our organisation
The Transplant Out-Patient Department delivers a range of outpatient services for a growing population of our pre and post- cardio thoracic transplant patients. This post will involve providing a wide range of clinical care and support to Transplant patients and their family and will also include some administrative duties.
The successful candidate will need to be flexible in their working practice and possess good communication and interpersonal skills. Previous Transplant experience, management of complex patients and proven clinical skills relevant to the out-patient environment are desirable.

1.Duties and Responsibilities
Clinical Care & Patient Advocacy
6. Assess, plan, implement, and evaluate programmes of care using evidence-based practice.
7. Take a holistic approach considering physiological, pathological, spiritual, socio-economic, and cultural factors.
8. Implement and regularly review nursing plans for assigned patients, ensuring individual pathways are followed.
9. Act as a patient advocate in line with the NHS plan.
10. Promote a healthy lifestyle for patients and staff in line with national strategies.
Documentation & Compliance
11. Maintain nursing documentation to Trust standards, including electronic records.
12. Participate in nursing audits to achieve agreed standards and meet CQUIN targets.
13. Record and report incidents/complaints in accordance with Trust policies and assist in investigations.
14. Ensure compliance with Trust policies, procedures, guidelines, standards, and the core behaviours and values
Leadership & Team Coordination
15. Regularly take charge of the clinical area, ensuring safe staffing levels and effective resource utilization.
16. Delegate tasks and activities to team members appropriately.
17. Collaborate with the Transplant Outpatient team to maintain departmental ethos and high standards of care.
18. Contribute to setting goals and benchmarking standards under supervision of senior staff.

19. Actively participate in clinical governance and patient safety walkabouts.
20. Identify and report clinical risks in line with Trust risk management policy.
21. Address verbal complaints promptly to resolve issues at a local level.

22. Provide a safe and suitable environment for the physical, psychological, and spiritual wellbeing of staff, patients, and relatives.
23. Work with the Departmental Manager to maintain safety and comfort in the clinical area.

37. Work together with the Transplant Outpatient team to ensure effective use and maintenance of resources and adequate stock levels.
38. Participate in planning the duty rota, considering the needs of the service and departmental parameters.
39. Actively engage with rostering in the Transplant Outpatient department and adopt a flexible approach to personal rostering to ensure service needs are met.
Service Development
40. Contribute to the development and maintenance of a positive learning environment for colleagues, patients, and visitors.
41. Participate in staff education and development, including orientation and supervision, and act as a mentor and resource for learners and the nursing team.
42. Be aware of service delivery/business plans.
Policy Development
43. Under the supervision of the department Sister/Charge Nurse, work on delegated projects relating to local policy development.
44. Actively participate in the implementation of Trust policies and procedures at ward/department level, reinforcing their use.

Person specification
EDUCATION & QUALIFICATIONS
Essential criteria
54. Registered Nurse with current NMC registration
55. Experience in mentorship, preceptorship, and staff development.
