Overview

The Norfolk Community Health and Care NHS Trust is seeking a dynamic Clinical Nurse Manager to join the Integrated Nursing and Therapy Urgent Community Response (UCR) team in South Norfolk. This role provides clinical leadership and line management to staff within UCR, supporting daily clinical delivery and operational management. It is suited to an experienced nurse with a background in acute, community, or urgent/emergency response settings. The closing date is 02 November 2025.


Main duties

Job Purpose

* Provide clinical expertise, clinical leadership and line management to staff working within Urgent Community Response (UCR).
* Support staff to achieve and maintain competencies within the service.
* Contribute to delivering high standards of healthcare to patients in their own homes.
* Ensure annual priorities and quality goals are embedded within the workforce, including completion of annual appraisals for the team.

Hours of Operation

* Service operates 08:00–20:00, seven days a week. Shifts will be within these times.


About us

The UCR Team covers all of South Norfolk. You will support staff across the South Norfolk area and contribute to service and quality development alongside the Clinical Operations Manager and UCR nursing and therapy leads. UCR provides admission-avoidance services via the Rapid Response MDP (formerly NEAT), the 2-hour UCR response referral process, referrals for end-of-life patients, and the discharge-to-assess pathway. UCR participates in innovative projects to keep patients out of hospital and cared for at home, including support for Virtual Ward patients and the Unscheduled Care Coordination Hub.

Job responsibilities

Main Duties and Responsibilities

* Provide professional and clinical leadership with a supportive, flexible leadership approach to promote confidence in the team and service.
* Clinically supervise and line-manage the UCR team, ensuring adherence to Trust values and the NMC code.
* Work with and deputise for the Service Lead as required.
* Oversee caseload management by monitoring and evaluating care plans to ensure quality care, progression, and timely discharge.
* Provide overview of remote monitoring systems within UCR and liaise with UCCH and VW Hub to support service delivery.
* Support the Clinician of the Day (COD) with task delegation and prioritisation based on clinical acuity.
* Work within the integrated team to prevent unnecessary hospital admissions and facilitate early discharge from hospital.
* Collaborate with healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service for service users.
* Support community clinicians and students through supervision, appraisal and mentorship in line with Trust policy.
* Ensure clinical supervision is occurring within the team and accurately recorded.
* Liaise with the UCR Clinical Operations Manager to identify management gaps and provide support as needed.
* As part of operational leadership for the locality, oversee allocation of work for clinicians and ensure competency with mobile working.
* Engage in triage and referral-to-discharge standard processes and operate to a demand-and-capacity model.

Clinical

* Oversee core clinical interventions: assessment, planning, implementation and evaluation of patient care, and development of care plans.
* Develop new skills in care delivery and share competencies with the wider team.
* Devise individualised care programmes after holistic assessments; adjust as required.
* Assess patients’ holistic needs and communicate complex information to patients and carers.
* Become a specialist in Assistive Technology and integrate it into patient care where appropriate.
* Adjust explanations to overcome barriers to understanding and obtain informed consent and concordance.
* Collaborate with UCR, UCCH, Virtual Ward clinicians, GPs, Social Services, inpatient teams, allied health professionals and the voluntary sector to meet identified needs.
* Provide patient-focused education, training and advice to patients, carers and professionals.
* Participate in multi-disciplinary/multi-agency meetings as appropriate (e.g., Gold Standard Framework).
* Plan and organise specialist services within the team; provide clinical advice to others, including Social Services and GPs.
* Record activity data and clinical information on SystmOne daily.
* Adhere to infection control policies and universal precautions; ensure informed consent is obtained prior to interventions.
* Maintain clinical credibility by acting as a clinical practitioner in own area of expertise.

Professional Leadership

* Undertake risk assessments and act on them appropriately; plan and prioritise workload to meet service targets.
* Maintain own and team paperwork/records; monitor equipment and supplies.
* Work with and deputise for the Service Lead; participate in integrated team meetings and chair meetings as necessary.
* Develop and maintain effective relationships with all colleagues and agencies; contribute to operational strategy and local projects.
* Support team members, ensure efficient use of resources, manage budgets, and contribute to recruitment and staff development.
* Attend required training; oversee safe use of equipment; remain accountable for professional actions.
* Engage in evidence-based practice, contribute to policy development, and provide induction for students and new starters.
* Mentor staff, provide training in specialist areas, and demonstrate leadership skills.
* Communicate management updates to the team regularly and assist in conflict resolution and complaint investigations as needed.
* Participate in staff appraisal and development planning; ensure adherence to the Trust behaviour framework.
* Support modernisation and changes within NCH&C, understand clinical governance and risk management, and monitor absence management and mandatory training compliance.
* Be an authorised signatory for goods, services and payroll; ensure timely HR documentation and data returns.
* Monitor delegated budget and assist Locality Management in resource management; manage absence, leave and training per policy.
* Ensure policies are interpreted correctly and implemented; line-manage UCR Band 6 and Band 5 nurses and other staff.


Qualifications

* 1st level degree or equivalent in Nursing
* RN with current NMC registration
* Recognised Management Qualification or evidence of working at that level
* Independent/supplementary prescribing qualification


Experience

* Experience at band 6 level within the community setting
* Evidence of strong clinical and professional leadership skills
* Experience of leading a team and managing budgets

* Highly motivated; able to provide high standards of care
* Tact, diplomacy and strong communication with patients, carers and teams
* Empathy, sensitivity and the ability to use own initiative

* Ability to travel to patients’ homes, clinics, base and meetings; comfortable working in cramped environments
* Ability to handle equipment and IT skills; keen observational and reporting abilities
