Buckinghamshire Mind is looking for an Information and Options Worker.

The Information and Options worker is responsible for raising patient awareness of services and activities in the community and encourage and support patients to use these services to improve longer term mental health and well-being.

The Information and Options Worker plays a key role in the Community Mental Health Team by working with patients to understand their longer-term recovery goals and connect them to services and activities in the community based on the individual’s identified needs and interests. The purpose of this role is to empower patients to engage with organisations and activities that aid their longer-term wellbeing and independence following discharge from CMHT.
* Contract Type: Permanent
* Hours: 37 hours per week (Monday – Friday)
* Location: hybrid working, with a minimum of two days in the High Wycombe centre and two in the Amersham centre, with frequent travel across South Bucks.
* Salary: £25,000

Key Responsibilities
* Working closely with the Chiltern Adult Mental Health Team the postholder will take referrals from patients across Oxford Health teams aged 18-65 for short term support and/or signpost to appropriate community support services – both within Bucks Mind and with other service providers.
* To work closely with patients to understand their recovery goals and provide ‘handholding’ support enabling individuals to attend the groups/activities in the community.
* Take referrals and support a caseload of patients through 1:1 support at both Saffron House and Amersham Health Centre, seeing patients either in their own homes or in community locations, and to provide details of the range of (support) options available in the community, signposting, referring or supporting as required.
The ideal candidate will have
* Experience working with at risk adults with mental health needs and be able to support individuals to achieve their goals in a person-centred way.
* A positive attitude towards people with mental health challenges, in-particular people’s ability to develop and change.
* Ability to respond appropriately to people in distress, maintain appropriate professional boundaries and build positive rapport.
* Ability to prioritise work appropriately, meet deadlines, keep timely appointments and manage a varied diary.
* Ability to travel around the area with access to a vehicle for use during working hours and with appropriate travel insurance (work related mileage will be paid).
