Qualifications:
1. Graduate of an accredited university with a B.S., M.A., M.S., or Doctorate in Occupational Therapy recognized by the AOTA.
2. SNF experience preferred
3. Holds current license and/or registration in the state (as applicable)
Job Responsibilities:
1. Provide comprehensive occupational therapy evaluations based on MD orders.
2. Document findings on standardized evaluation forms accurately and timely, following all guidelines.
3. Develop comprehensive treatment plans with goals, frequency, duration, and modalities.
4. Obtain MD orders for evaluations, treatments, recertifications, and discharges.
5. Provide treatment using modalities, modifying as needed, while adhering to precautions.
6. Accurately document patient participation and progress daily.
7. Update treatment plans monthly and document recertifications promptly.
8. Complete discharge summaries and in-service forms as indicated.
9. Complete all required documentation forms.
10. Contribute to care plans and MDS documentation as needed.
11. Attend facility and rehab meetings as directed.
12. Participate in facility committees.
13. Act as a rehab delegate to the interdisciplinary team.
14. Assist in scheduling patients and perform screens as indicated.
15. Bill for occupational therapy services ethically and accurately.
16. Cover colleagues' caseloads during absences if needed.
17. Supervise junior staff, students, and technicians; co-sign notes as required.
18. Maintain professional relationships with staff, patients, and families.
19. Provide in-service education on patient care and therapy services.
20. Follow all facility policies, procedures, and confidentiality regulations.
21. Report to work punctually, adhere to scheduled hours, and maintain professionalism.
22. Meet productivity standards and comply with HR policies regarding leave and overtime.
23. Foster a positive work environment and perform other duties as assigned.
