Overview

Join Home 1st as a Band 3 Rehabilitation Assistant – Help to Shape the Future of Community Care. This role is in Milton Keynes and involves community and inpatient rehabilitation, working with Physiotherapists, Occupational Therapists, Nurses, and a multidisciplinary team to help people stay independent and safely return home after illness or injury.

Responsibilities

* To undertake specific support work to aid rehabilitation of service users, in the achievement of optimal functional independence, with or without the assistance of others.
* To provide flexible programmes and interventions of therapeutic activities, as directed by a registered member of staff.
* To work without direct supervision, monitoring progress and minor adaptations to interventions as required, under the guidance of a registered member of staff.
* To communicate effectively and work collaboratively with all members of the multidisciplinary team and other services within Home1st, as well as outside agencies.
* To work within any of the Home 1st Planned Care Therapy pathways and participate in rotation to different therapy teams as required.
* Participate in development of integrated pathways.
* To be responsible for the ordering and provision of Allied Health Professional or Band 4 Rehabilitation Assistant prescribed equipment.


Clinical Duties

* To provide physiotherapy, occupational therapy and nursing interventions as part of a specific treatment plan agreed and documented by physiotherapist/occupational therapist/nurse.
* To undertake tasks in delivering patient care, rehabilitation or social care as directed by the service professionals in a range of settings.
* To work independently in the designated clinical area/patients home following an agreed treatment plan.
* To undertake support work with patients individually or in a group with or without supervision of a physiotherapist or occupational therapist.
* To follow departmental and trust guidelines for obtaining consent and use diplomacy and gentle encouragement to enhance patient’s compliance in treatment programmes.
* To work with family members, carers and support workers to enable them to carry out treatment programmes planned by the physiotherapist, occupational therapist or nurse.
* Manage own workload allocated by the physiotherapist, occupational therapist or nurse.
* To work as a member of the multidisciplinary team and liaise with the team effectively to ensure co-ordinated approach to patient care and attend meetings as delegated by the nurse, physiotherapist or occupational therapist.
* Maintain comprehensive and accurate records of work undertaken, including documenting in line with Trust and service standards.
* To attend in-service training, clinical supervision and team meetings as required.
* To assist physiotherapist, occupational therapist or nurse with complex patients in their own home environment.
* Have the ability to handle patients with mobility problems, in an enabling way (e.g., use of hoists, sliding sheets, wheelchairs, mobility aids).
* To actively participate within the completion of Band 3 level competencies.


Effort

* To carry out interventions with patients with a variety of conditions, as directed by the AHP, with frequent episodes of low to moderate physical effort and occasional episodes of moderate to intense physical effort daily (e.g., transfers, hoists, handling techniques, transporting patients).
* To comply with CNWL-MK Manual Handling Policy and local therapeutic handling guidance at all times.
* Work may involve cramped conditions; physical requirements include repetitive movements, standing/walking for most of the day, and occasional bending, crouching, kneeling, twisting and crawling.
* To deal sensitively with patients and carers who may have high levels of anxiety, fear or aggression due to various conditions, reporting any incidents per CNWL-MK policies.
* Frequent exposure to unpleasant working conditions and occasional exposure to unsanitary home conditions; maintain safety and use protective equipment as required.
* To use mental effort for assessment, treatment, and documentation; to develop therapeutic relationships with patients in difficult circumstances.
* To interact appropriately with patients and relatives experiencing challenging circumstances and to facilitate intervention as directed by the Clinician.


Organisational Duties

* Maintain equipment and report defects or hazards; manage own caseload within scope of competence.
* Maintain up-to-date electronic diary and outcomes of appointments.
* Ensure safety of the designated clinical environment and awareness of emergency procedures.
* Assist in risk assessments and incident reporting; adhere to appraisal and performance review processes.
* Participate in data collection and statistics as directed; attend mandatory training as required.


General Duties & Responsibilities

* Assist in simple administrative procedures as required by the Team Lead.
* Ensure effective communication with the nurse, physiotherapist or occupational therapist regarding treatment.
* Comply with organisational and departmental policies and procedures.
* Perform general administrative tasks (photocopying, data input, filing, telephone duties) in a courteous manner.
* Maintain accurate and comprehensive clinical records and documentation processes.

* Follow relevant national and local codes of practice; maintain professional registration where required.
* Protect confidentiality and handle information according to data protection and security policies.

* Attend mandatory and essential training to ensure safe and efficient functioning.
* Participate in annual appraisal and development planning.
* Comply with infection prevention and control policies and training requirements.
* Safeguard and promote welfare of children and vulnerable adults; adhere to guidance and training requirements.

* Familiarize with local policies and access to intranet or seek line manager guidance if needed.
