What you will do:
Conduct assessments to understand and support the needs of adults within the South Neighbourhood Team
Deliver comprehensive Care Act duties to ensure the wellbeing and autonomy of service users
Collaborate effectively with colleagues and external partners to provide holistic care and support
Utilise your experience in Adult Social Care and Liquid Logic systems to maintain detailed and accurate records
Facilitate and coordinate care plans, ensuring they are tailored to the individual's unique needs
Advocate for individuals, ensuring they have a voice within the care system
