What you will do:
* Deliver a high-quality social work service that meets the needs of adults, carers, and their families in a timely manner.
* Conduct assessments and care planning, ensuring adherence to statutory guidelines and the Care Act’s wellbeing principles.
* Collaborate with service users, their carers, and multi-disciplinary partners to manage caseloads and conduct robust assessments.
* Develop outcomes-focused support plans that promote personalised support, choice, and control for service users.
* Facilitate service user engagement in identifying asset-based goals and developing strategies to meet longer-term needs and aspirations.
