Contract details
The post is a permanent contract, full time at 37hrs per week. Salary: £39,862 - £44,075 (dependant on experience) including annual increments. You will be based at Eddie Ferguson House, Blyth but we offer flexible working options, including the option to hybrid work from alternative locality bases around the County. Responsibilities
As a Social Worker in our Learning Disability team, you will undertake care and support needs assessments, care planning and reviews, complete Care Act and Mental Capacity assessments, and work closely with individuals, families and carers to create tailored care plans that truly make a difference. You will also collaborate with internal and external statutory, private, charitable and voluntary stakeholders to ensure residents receive the right support and resources to achieve better outcomes. You will enjoy autonomy in managing your caseload while working closely with a supportive team and receiving guidance from Advanced Social Work Practitioners and your Team Manager.

Qualifications
* Degree in Social Work & registered with Social Work England
* Successfully completed the UK Skills for Care ASYE programme
* Experience in Social Work with adults and experience working within Adult Social Care teams, particularly with clients whose primary need is a learning disability
* Valid UK driving licence and ability to use a car for work purposes
