Discharge Facilitation and Intervention Autism Practitioner

An exciting opportunity for a Band 6 practitioner to join the Specialist Autism Team. The team works closely with secondary mental health services to prevent unnecessary hospital admissions for autistic individuals and delivers a person‑centred, bespoke service.
Main duties of the job
The post holder will:
• Hold a caseload of patients to facilitate a timely discharge from mental health hospital.
• Deliver autism‑specific interventions, offering advice and guidance to clinical teams and sharing information with those most important to the patient.
• Work flexibly between 9 am and 5 pm, Monday to Friday, travelling across the LSCFT footprint to patients in community and inpatient settings.
• Contribute to multidisciplinary team development sessions, sharing knowledge and information.

Person Specification
Qualifications
* Registered nurse (RLDN, RMN), OT or social worker.
* Post‑qualifying experience within the NHS or local authority.
Knowledge
* Knowledge and skills in the assessment and management of risk.
* Demonstrates continuing professional development within mental health, learning disability or autism.
* Knowledge of legal frameworks applicable to autistic people.
* Knowledge of contemporary autism policy.
Experience
* Ability to support autistic people on an individual and group basis.
* Experience working with individuals who present behaviours that may challenge services.
* Experience in statutory community or inpatient settings.

Salary: £39,959 to £48,117 per annum (subject to confirmation).
