What you will do:
* Assess and develop treatment plans to promote patient recovery within the Intermediate Care Team.
* Collaborate with multidisciplinary teams to tailor interventions and enhance patient outcomes.
* Provide advice and support to patients, carers, and families to encourage progress within care plans.
* Monitor and review patient progress and adapt treatment accordingly.
* Document patient progress and treatment effectiveness, ensuring accurate and detailed records are maintained.
* Engage in team meetings and professional development sessions to support continuous service improvement.
