Health Care Support Worker\

Employer: The Confederation, Hillingdon CIC – a not‑for‑profit community interest company working with General Practice and other healthcare providers to deliver excellent clinical services and support GP practices across London.

About the role: A Healthcare Support Worker will work within a multi‑professional team in a GP setting, providing support, care and treatment to patients under the guidelines and protocols identified for the role.


Primary Responsibilities

* Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management and lifestyle interventions.
* Plays a key role in vaccination and immunisation programmes within the PCN.
* Works within a Multi‑Disciplinary Team to support patients with long‑term conditions and health inequalities.
* Provides clinical support to registered nurses and other healthcare professionals within the PCN.
* Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives.
* Records vital signs – blood pressure, height and weight.
* Performs ECGs and phlebotomy.
* Assists with chronic disease monitoring.
* Conducts health checks.
* Assists GPs and nurses with minor procedures.
* Updates patient records and ensures accurate documentation.

* Must hold a care certificate or be working towards one.
* Participates in an annual individual performance review and maintains a record of personal and professional development.
* Demonstrates self‑motivation, adaptability and takes ownership of learning.
* Keeps the lead GP informed of any concerns regarding role requirements and professional development needs.
* Respects professional boundaries and acts appropriately when limits are reached.


Quality

* Maintains quality within the practice and alerts team members to quality and risk issues.
* Follows practice protocols, assesses own performance and takes accountability under supervision.
* Contributes to team effectiveness by reflecting on activities and suggesting improvements.
* Manages own time, workload and resources effectively.


Contribution to the Implementation of Services

* Applies Practice policies, standards and guidance.
* Discusses with team members how policies, standards and guidelines affect own work.
* Participates in audits where appropriate.


Health and Safety / Risk Management

* Promotes and maintains health, safety and security for self and others in accordance with the Practice Health & Safety Policy.
* Uses personal security systems and manages risks in work activities.
* Utilises training to update knowledge and skills and follows infection control procedures.
* Reports potential risks and complies with local Health & Safety policies.
* Complies with the Data Protection Act (1984) and the Access to Health Records Act (1990).

Communication & Working Relationships

* Communicates effectively with team members, patients, carers and outside agencies.
* Familiar with all communication mediums (email, tasks, telephone, etc.).
* Responds appropriately to alternative communication needs.
* Maintains effective communication pathways with the project team.


Special Working Conditions

* Travels independently between sites (where applicable) and attends meetings hosted by other agencies.

Person Specification


Essential Criteria

* GCSE Grade A to C in English and Maths.
* Has or is willing to work towards a care certificate or equivalent qualification.
* Experience coordinating tasks and liaising with multiple healthcare professionals.
* Experience using technology and digital tools.
* Self‑motivation.
* Adaptability.
* Immunisation status – HEP B / MMR.


Desirable Criteria

* Primary care experience as a Healthcare Assistant, Practice Administrator similar.
* Experience working with NHS or social care professionals.
* Experience using clinical systems such as EMIS.
* Full driver’s licence.
