Primary Care Network (PCN) Mental Health & Wellbeing Coach

Unfortunately NPC does not hold a license for Visa Sponsorship and cannot progress any applications for candidates without the right to work in the UK.

Hours: 28 hours per week

Salary: Up to £23,900.27

Place of work: Practices in NN4, aPCN in North Norfolk - Acle Medical Partnership, Brundall Medical Partnership, BlofieldSurgery, Hoveton & Wroxham Medical Centre, Staithe Surgery, Ludham &Stalham Green Surgery.

The post holder is a Mental Health & Wellbeing Coach, who worksalongside the general practice team and a mental health multi-disciplinary team,providing non-clinical coaching and wellbeing support only within NN4 PCN.

The individual will act within their professional boundaries to work forthe benefit of patients, providing coaching support to improve wellbeingwithout providing clinical treatment.

They will provide assistance and advice to other members of the primaryhealthcare team in managing the mental wellbeing of the practice population.


Main duties of the job

SpecialRequirements of the Post:

* Participate in regular individual and groupcoaching-focussed supervision with a suitably qualified supervisor.
* Develop collaborative relationships and work inpartnership with health, social care, community, and voluntary sector providers,as well as multidisciplinary teams, to support patients wider health andwellbeing and contribute to reducing health inequalities.
* Provide education and specialist expertise toPCN staff, supporting them to improve their skills and understanding ofpersonalised care and behavioural approaches, and ensuring consistency in thefollow-up of patients goals with MDT input.
* Raise awareness within the PCN ofshared-decision making and decision support tools.
* Demonstrate understanding of safeguardingprocesses for vulnerable people.

Job responsibilities

* Work as part of a multi-disciplinary teamin a first-contact patient facing coaching roleto providepersonalised support to individuals, their families, and carers to empower themto manage their own wellbeing and live independently.
* Criteriaincludes supporting individuals experiencing challenges affecting wellbeing(e.g. anxiety, stress, low mood, life events). The role does not provideclinical treatment, diagnosis, or therapy.
* Engage patients in coaching, motivating them throughmultiple sessions to identify their needs, set goals, and achieve theirpersonalised wellbeing objectives.
* Provide non-clinical interventions, including self-managementeducation and peer support.
* Support personal choice and encourage steps to improve wellbeing, without making clinical risk decisions.
* Work in partnership with social prescribingservices to connect patients to community-based activities or support charities,enabling them to take greater control of their wellbeing.
* Manage and prioritise a caseload of patients, takinga non-judgemental, person-centred approach that emphasises communication.
* Refer patients to other health professionalswithin the PCN when needs exceed coaching scope (e.g., GP, social prescriber,or mental health practitioner). Referrals should not include private services.The coach does not provide therapy or manage clinical cases.


Person Specification


Experience

* Minimum of 2 years experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field.
* An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing.
* Experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
* An appreciation of the new NHS landscape including the relationship between individual providers, PCNs and commissioners.
* Experience of supporting individuals with mental health and wellbeing needs.
* Understanding of evidence-based wellbeing and behaviour change approaches.
* Understanding of the mentorship process.


Skills and Abilities

* Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.
* Ability to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
* Excellent interpersonal, influencing, negotiation, and organisation skills, with the ability to constructively challenge views and practices in a professional and supportive manner.
* Ability to work under pressure and to meet deadlines.
* Ability to maintain confidentiality.
* Ability to use own initiative, discretion and sensitivity
* Full driving license.


Qualifications

* Enrolled in, or qualified from appropriate health coaching training.
* Self-development through continuous professional development activity.
* Willingness to undertake further study relevant to mental health in general practice supported by GP clinical supervision.
