LOCATION: Community based across the Bradford District GRADE / SALARY: D1 £26,244.40 FTE per annum HOURS: 37.5 hours per week ACCOUNTABLE TO: Team Leader (REACH) Purpose of the Job: To work as part of the Reach team and alongside the Community Mental Health Teams (CMHTs) to support clients with serious mental illness (SMI) to reach their goals. Main Duties: Hold a caseload of clients, providing one to one support for individuals in a recovery-focused and person-centred way, which builds confidence and helps people to move forward. Use your own lived experience of recovery from mental health problems, as appropriate, to inspire and support others. Ensure that client notes are recorded in an accurate and timely manner and kept updated. Work in line with all relevant safeguarding policies and procedures, ensuring concerns are escalated appropriately. Work closely with other health and social care professionals including statutory and voluntary agencies to ensure that support is joined up and of high quality. Person Specification Listed below are the knowledge, experience skills and values you’ll need to do this job, we will assess these through your application or through tests or interviews after shortlisting. Knowledge Method Educated to GCSE Level 4 or above, or equivalent experience Application Awareness or knowledge and/or lived experience of the challenges facing people who have difficulties relating to their mental health. Application Assessment Experience At least 2 years’ experience of working one to one with clients who face challenges with their mental health, including with clients who have SMI.
